Study: Biopsy-proven Inflammatory Dilated Cardiomyopathy after COVID-19 Jab

Simian Practicalist

A Japanese paper by K. Hashimoto et al titled “A case of biopsy-proven inflammatory dilated cardiomyopathy following heterologous mRNA-1273 third-dose immunization” published on 1 July 2024 reports the first case of the aforementioned.

The paper is 8 pages long.

The patient is a 78-year-old previously healthy female. She had received the Pfizer jabs for her two-dose primary course. The third dose was Moderna.

On the fourth day after vaccination [third dose], the patient experienced palpitations and dyspnoea, which gradually worsened. The patient had a history of mild dementia but no risk factors for coronary artery disease.

She was admitted to the hospital 11 days after receiving the third dose.

The T2-weighted image revealed the absence of remarkable myocardial oedema, suggestive of dilated cardiomyopathy (DCM) (Figure 3). However, the DCM phenotype following COVID-19 immunization prompted us to perform an endomyocardial biopsy (EMB) (Figure 4A–E). Photomicrographs showed moderate myocyte hypertrophy, interstitial fibrosis and scattered interstitial inflammatory infiltrates, largely composed of CD68+ macrophages admixed with CD3+ T-lymphocytes, without associated myocyte necrosis.

The patient was successfully treated with corticosteroids.

Although the possibility SARS-CoV-2 infection could not be completely ruled out, this is unlikely as “the patient had no recent travel history nor any contact with patients with confirmed COVID-19, including family members”.

Also, “the levels of serologic viral antibody titres increase four-fold or more during acute infection”. However, “no significant increase was observed in the levels of paired viral antibody titres and no clinical signs and symptoms suggestive of acute viral infection were present” so active viral myocarditis is unlikely.

In short, the problem can be attributed to the so-called vaccine.
